How to Change Your Name, Address, or Phone Number in Florida

I have a name change; what do I do now?
If you lawfully change your name for any reason, you are required by law to notify the Florida Department of State’s office within 60 days of the change.
First, contact your notary bond agent to request the Amended Commission Request Notice of Name Change Form. Send the name change form, fees, new stamp order form, and your current commission certificate to your bond agent. If you do not have your current commission certificate, that is okay; write a short letter explaining why you no longer have your certificate.
Your bond agency will forward all necessary forms to the state for you. You may continue to use your current stamp until you receive your new notary stamp and commission certificate (these generally will come separately in the mail). This process typically takes 2-3 weeks.
I have a change of address and/or phone number; who do I notify?
If you have a change of address, either business or residential, you must notify the Florida Department of State’s Office, in writing, within 60 days of the change. This also applies to a change in phone number. The address is:

You should also keep your information up to date with your bond agency as well.
If you have moved out of the state of Florida, you must resign your notary commission. (The state of Florida requires you to be a Florida resident to have a Florida notary commission.)
